a. Name of a person with whom Electronic Communication Services (ECS) can work to coordinate the installation.
b. Signature of individual authorized to charge against departmental funds with account/fund/sub to be charged.
c. The name of the actual user of the data circuit.
d. The @uci.edu electronic mail address of the person using the circuit.
e. Check box which describes the type of data service:
1) Asynchronous - RS232 - Slow speed (9600 bits-per-second)
Access to campus computers is provided by terminal servers and the campus data network.
2) Ethernet - High speed (ten million bits-per-second)
ECS provides a connection to a Multiport repeater (MPR) unit. Access to campus is provided if the MPR is connected to network by way of a router interface. Users who are requesting Ethernet service for the first time should contact Electronic Communication Services at 824-2222 for further details.
3) PhoneNet - Medium speed (230,400 bits-per-second) for Macintoshes.
Access to campus computers requires that an Ethernet-to-AppleTalk gateway be purchased in addition to a router interface card or an Ethernet connection to an existing campus-connected subnet.
Attach to the request form a room layout with new location of data line designated by a small triangle. All devices, outlets, and cabling should be available at all times. Send request form to Electronic Communication Services, Zot Code 5475, or fax to 824-2270.
Requests which are incomplete will be returned to the originator; therefore, if there are questions about a service request, call 824-2222.
a. For installation of one to ten data lines in a department, allow ten working days from the time the request is received by ECS.
b. Data communications installations involving more than ten data lines require longer intervals. Contact Electronic Communication Services, 824-2222, for an estimate of the time required.
All data communications outlets and cable facilities must be accessible at all times for installation and repair. Bookcases, ceiling tile, file cabinets, and all other furniture blocking access to facilities must be moved by the requesting department.
1) Station ID or jack outlet number of data line to be worked on or moved.
2) Locations affected by change; i.e., present location of data line, room and building, and new room and building location to which data line is to be activated. If the data line is to be moved within the same room, indicate the data line move as the "same room."
3) Name and extension of a person familiar with the work requested whom the installer can contact for proper placement of data line.
4) Signature of individual authorized to charge against departmental funds with account/fund/sub to be charged.

Do not ask the data line installers to make changes to the order as they can do only what the order specifies.
1. Circuit ID or jack outlet number of data line to be disconnected and date of last day service is required. Disconnects cannot be retroactive.
2. Signature of individual authorized to charge against departmental funds with account/fund/sub to be charged.
Send request form to Electronic Communication Services, Zot Code 5475, or fax to 824-2270.
Requests which are incomplete will be returned to the originator; therefore, if there are questions about a service request, call 824-2222.
2. Departments have 30 days to request an adjustment to their bill. After that time, no adjustments will be made.
3. For billing questions, call 824-4311.
